In accordance with the Administrative Rules for the Michigan Medical Marijuana Program, medical marijuana patients in the state can obtain replacement medical marijuana cards. Patients may require replacement medical marijuana cards if their current cards were stolen, destroyed, or lost.
Although there is no specific requirement to notify the Cannabis Regulatory Agency when a medical marijuana card is stolen or lost, it is recommended that you contact the agency by calling (517) 284-8599 to report the incident.
You may request a replacement medical marijuana card by mail or online. To complete a request by mail, fill out and submit the completed Request Replacement Card Form and your valid Michigan driver's license or personal identification card to:

You may also provide a signed voter registration instead of a Michigan driver's license or personal ID card. If you do so, you must also submit a copy of a government-issued document, stating your name and date of birth, to confirm your identity.

The Michigan Cannabis Regulatory Agency does not charge a fee to obtain a replacement card.
